Four important questions for everyone with chronic insomnia

When I talk to people who are struggling with insomnia, I typically ask them four questions to help me identify the perpetuating factors behind their insomnia. These questions are:

1. When do you normally go to bed at night?
2. When do you normally get out of bed to start your day?
3. How many hours of sleep do you get on a typical night?
4. Why do you think you struggle with sleep?

These questions are so helpful because they help identify which of the three perpetuating factors for insomnia are present. These three factors are:

1. Sleep drive disruption (normally occurs if we go to bed before we are sleepy enough for sleep, spend too much time in bed, and/or nap during the day)

2. Body clock disruption (normally occurs if we get out of bed at a different time every day and/or we are mainly inactive during the day)

3. Arousal (this can be conditioned arousal (associating the bed with wakefulness, anxiety, worry, and frustration), physiological arousal (a racing heart or surge of energy when in bed), and/or hyperarousal (typically a racing mind that won’t shut down at night)).

When we identify the perpetuating factors behind chronic insomnia, we can then take steps to address them to improve sleep for the long-term.

Cognitive behavioral therapy for insomnia (CBT-I) techniques specifically address the perpetuating factors of chronic insomnia — and this is exactly why they are so effective.
